The question really is - how hard is it to change to an indexed printed document?

This is quite a big area, of course, but have a look at Codev.TWikiForBookAuthoring and be sure to hit the Ref-By link at the bottom for some other discussion. The short answer is that it involves some Perl/TWiki hacking to integrate with other tools and formats such as SGML, DocBook and PDF.
