Plugins Survey Ideas

I want to survey plugins users to find out where plugins are used. I've prepared a page, not active yet, that I'd like to use: see ComponentSurveyDraft (incomplete).

As well as this one-off survey I'd like for us to add a %COMMENT box to the template for *Plugin, *Skin and *Addon pages that lets users add themselves to the survey if they pick up the plugin later.

To do this properly requires the addition of the "poll" user-defined comment template from the latest revision of the comment plugin (not currently installed at twiki.org).

Doesn't this acheive the same goals as the PluginAppraisalReport?
