SID-01762: Item does not reflected in ItemToDo

I have followed all the step from http://twiki.org/cgi-bin/view/Sandbox/ItemToDo for creating to do list. The form is created and configured properly, but when I submit the form the item does not reflected in item list or ItemDone page.

The new item entry comes under the child topic section of ItemToDo page. Search macro does not shows added item.
