Question

Since the update from TWiki 4.0.1 to 4.0.2 we are missing the wysiwyg button at the header of each page. The plugin is enabled in the configuration section and there is no error message in the diagnostic section of this plugin. Any idea ?

Answer

Patternskin changed from a table based layout to a div-based going from 4.0.1 to 4.0.2.

This sounds like your skin was not fully upgraded in the process, you should do an extra check, that all files are 4.0.2 version.
