Plugin packages should, IMO, never package the ,v file because otherwise when you install them, they wipe out any site settings on install and they become irretrievable from the Page History

Ouch. Something like TWikiTopicUpgradeScript would be really useful in preventing this.

I agree, and for this reason and others I have never shipped ,v files with my plugins. The master of the plugin topic file should be revision controlled in CVS, like the rest of the plugin.

That needs to be thought through carefully. Simply copying over only the topic text file will result in inconsistent text and revision files. An installer could bump up the revision of an existing topic on upgrade.

When I upgrade a Plugin I usually copy the topic text manually into the edit box of the browser. That way I get the correct revision history.
